Job SummaryWe are seeking a skilled Registered Nurse to join our HomeCare & Hospice team. As a Registered Nurse, you will provide high-quality patient care in the comfort of patients' homes, working closely with interdisciplinary teams to develop and implement individualized care plans.
Key Responsibilities- Assess patient needs and develop comprehensive care plans in collaboration with healthcare teams.
- Provide direct patient care, including medication management, wound care, and education on disease management and self-care.
- Collaborate with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
- Document patient care and services provided in accordance with Duke Health policies and regulatory standards.
- Participate in interdisciplinary team meetings and case conferences to ensure comprehensive care and service delivery.
- Current RN license in NC or compact RN licensure from participating state.
- Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
- 1-2 years of med-surgical, critical care, or home health/hospice/infusion experience preferred.
- Valid driver's license and current automobile insurance coverage.
