Key Responsibilities:
  • Coordinate all admissions into the Care Center, ensuring timely and accurate processing of resident information.
  • Act as the liaison between external referral sources, internal transfers, and Care Center departments, facilitating effective communication and collaboration.
  • Complete Medicare documentation and other admission-related paperwork, ensuring compliance with regulatory requirements.
  • Develop and maintain relationships with referral sources, promoting our Care Center's services and reputation.
Requirements:
  • 1-3 years of experience in admissions or a related field, preferably in a skilled nursing facility.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with stakeholders.
  • Knowledge of Medicare requirements and regulations, with the ability to apply this knowledge in a fast-paced environment.
  • Organized and accurate, with the ability to multitask and prioritize tasks effectively.
  • Proficient in Microsoft Office, with the ability to learn and adapt to new systems and technologies.
