Your Responsibilities:


With a designated roster of patients, you will deliver ongoing assessments and end-of-life nursing care in homes and skilled nursing facilities across various counties.

This includes formulating care plans, administering treatments, and collaborating closely with patients and their families. You will also maintain precise patient records using mobile technology.

Qualifications:

Current Ohio RN nursing license and a minimum of one year of acute care or medical-surgical experience.

Experience in a Medicare-certified home health agency and familiarity with electronic medical records (EMR) is preferred.

Reliable transportation (you will utilize your own vehicle), a valid driver’s license, insurance, and a satisfactory driving record are required.
