Department Details
- Department: Case Management - Emergency Department
- Schedule: 4x10 with Rotating weekends - 1:00pm - 11:00pm
- Hospital: Ascension Sacred Heart Hospital - Bay Medical

Key Responsibilities
The Case Management Registered Nurse will:
- Coordinate the comprehensive care plan for patients from admission through discharge.
- Develop a care strategy that reflects patient and family preferences and values.
- Oversee patient care and advocate for necessary resources.
- Maintain effective communication with healthcare providers to ensure successful implementation of care plans.
- Act as a resource for medical documentation and treatment alternatives.
Qualifications
Licensure / Certification / Registration:
- Must possess a valid Registered Nurse license from the Florida Board of Nursing.
Education:
- Diploma from an accredited nursing school or required professional licensure at the time of hire.
Preferred Experience
We prefer candidates with at least one year of experience in case management.
