Job Overview

PanCare of Florida Inc is seeking a skilled Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring our patients receive the highest standard of care.

Key Responsibilities:

  • Conduct thorough assessments of patient needs and develop personalized care plans.
  • Coordinate and facilitate access to medical, behavioral health, social, and educational services.
  • Monitor patient progress, make adjustments to care plans as needed, and ensure seamless transitions between services.
  • Collaborate with patients, families, and healthcare providers to achieve optimal health outcomes.
  • Maintain accurate and confidential patient records, adhering to all relevant regulations and standards.

Requirements:

  • Bachelor's degree in social or behavioral sciences.
  • Knowledge of community-based behavioral health care and case management principles.
  • Excellent interpersonal and communication skills.
  • Current Basic Life Support and Cardiopulmonary Resuscitation Certificate.
