The Position:

We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality health services and direct care to individuals with disabilities living in a residential program.

Key Responsibilities:
  • Provide specialized nursing and day-to-day care to individuals with disabilities
  • Document health care services in accordance with state regulations, nursing standards, and Massachusetts LPN license requirements
  • Act as a liaison with community health care providers, monitoring medical records and medication
  • Collaborate with the Program Director and registered nurse to assess, organize, and maintain health care and medication needs
  • Provide in-service education to individuals and/or staff on health issues
  • Administer medications to assigned individuals following Department of Public Health Medication Administration Program (MAP) policies and procedures
Requirements:
  • Graduation from an accredited and approved licensed practical nurse program
  • Current Massachusetts Nursing license
  • Two years general nursing experience and/or service to individuals with developmental disabilities in a community health setting preferred
  • Experience working with adults with DD and behaviors is a plus
  • Must have a Driver's license for at least one year with a good driving record history
  • Must be able to drive a 12 passenger van
