Job SummaryThe Licensed Practical Nurse (LPN) will provide health services and direct care to individuals with disabilities living in a residential program. The incumbent will also provide advocacy and liaison with community healthcare providers, monitor medical records, and administer medications.
Responsibilities- Provide direct specialized nursing and other day-to-day care to individuals.
- Document healthcare services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements.
- Function under the supervision of the residential manager, in collaboration with the registered nurse and Director of Health Care.
- Act as a liaison as needed to other community agencies and treatment providers regarding regular medical needs and care.
- Help staff identify appropriate community health resources.
- Provide in-service education to individuals and/or staff on health issues.
- Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures.
- Administer medications to assigned individuals following MAP policies and procedures.
- Provide quality assurance reviews of medication systems and MAP compliance.
- Work collaboratively with the residential manager and registered nurse evidenced by meeting weekly or as needed for medical updates.
- Work collaboratively with the nursing/healthcare staff of Bridgewell by participating in monthly nursing staff meetings and agency-wide healthcare planning.
- Collaborate with the residential manager and registered nurse to assess, organize, update, and maintain healthcare and medication needs.
- Graduation from an accredited and approved licensed practical nurse program.
- Current Massachusetts Nursing license.
- Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.
- Experience working with adults with DD and behaviors is a plus.
- Must have a Driver's license for at least one year with a good driving record history.
- Must be able to drive a 12-passenger van. We will provide training.
