Job Summary

The Physical Therapist will be responsible for evaluating, planning, and implementing specific treatment programs for individual patients according to the principles and practices of physical therapy. This will involve working as an interdisciplinary team member to provide patient assessment/re-assessment, direct patient care, and individualized care planning/implementation.

Responsibilities
  • Evaluate patients to determine their physical therapy needs and develop individualized treatment plans
  • Implement treatment programs to improve patients' physical function and mobility
  • Conduct patient assessments and re-assessments to monitor progress and adjust treatment plans as needed
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans
Requirements
  • Bachelor's Degree from an Accredited Program in Physical Therapy
  • Licensure and Registration in Arizona
  • CPR Certification
  • Excellent customer service skills
  • Strong interpersonal skills to interact positively and effectively with patients, families, and physicians
