Their services encompass primary care, chronic disease management, psychiatric support, post-acute care, transitional care, and remote patient monitoring.
POSITION DESCRIPTION:
The Nurse Practitioner will act as the primary care provider for a specific group of geriatric patients. All patients are situated in facilities or private residences. Depending on the assigned territory, the practice can offer various combinations of facilities (ALFs / ILFs / SNFs) and home-based patients. During the initial interview, the practice will assess the provider's location and discuss this in detail.
In skilled nursing facility settings (with no daytime travel), providers typically see an average of 15 to 16 patients daily.
When traveling between assisted living facilities, independent living facilities, and homes, providers generally see an average of 9 to 12 patients per day, adjusted based on actual travel time.
Key Responsibilities:
- Conduct health assessments to identify patient needs
- Develop personalized care plans
- Regularly evaluate patient conditions and modify treatment plans as necessary
- Administer medications, treatments, and other therapeutic interventions as prescribed
- Perform minor procedures and wound care as required
- Monitor vital signs and patient responses to treatment
- Educate patients and families on health management, disease prevention, and treatments
- Provide guidance on lifestyle modifications and self-care practices
- Collaborate with physicians, nurses, and other professionals to coordinate comprehensive care
- Ensure compliance with all relevant healthcare regulations and standards
On-call responsibilities involve phone triage only, with minimal volume. Providers rotate on-call duties, with a shift every 10 weeks or less for full-time providers. Part-time providers will engage in on-call duties on a negotiated schedule.
REQUIREMENTS:
- Ohio NP license
- DEA certification
- 1+ year of advanced practice experience
- Previous experience in geriatrics, long-term care, skilled nursing facilities, or house calls
