Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Dayton Children's. As a key member of our pharmacy team, you will be responsible for overseeing the daily operations of our outpatient pharmacy, ensuring the safe and effective processing and delivery of medications to our patients.

Key Responsibilities
  • Medication Order Review
    • Accurately review prescriptions and medication orders, checking medication for correct drug, route, dose, patient, and frequency.
    • Calculate dosage based on mg/kg and verify patient allergy information prior to dispensing medications.
    • Intervene with physicians when medication order is not appropriate for the patient. Document intervention in computer system or on hardcopy of prescription.
    • Dispense medication safely and accurately for the patient. Appropriately label all medications prior to dispensing.
    • Prioritize orders received in pharmacy based on clinical status of the patient.
    • Prepare medications in a timely manner and adhere to department turnaround times for routine and STAT orders/prescriptions.
  • Checks Medications Prior to Dispensing
    • Accurately check medications prepared by the technician for correct drug and dosage prepared.
    • Check compounded medications for accuracy and pharmaceutical appropriateness.
    • Monitor the activities of the technician to ensure that products are prepared safely and accurately.
    • Ensure the work area for preparing medications is clean, disinfected, and free of unrelated supplies and equipment.
    • Check medications prior to being refilled in the Pyxis Med stations.
    • Check medications sent via courier for accuracy prior to initiation of delivery.
  • Drug Information
    • Knowledgeable of the drug information resources available, including LexiComp and Micromedex.
    • Provide accurate drug information to healthcare providers when called upon.
    • Make clinical decisions based on drug information provided.
    • Access laboratory values for specific patients to aid in dosage decisions.
    • Answer phone promptly and field questions when appropriate.
    • Utilize the Medication Resource Center for additional information.
  • Outpatient Pharmacy
    • Utilize computer system to check patient profile for drug utilization.
    • Demonstrate the ability to enter a prescription into computer system, check voice mail line, and update physician information.
    • Compound medications using established recipes and proper 795 guidelines.
    • Demonstrate broad knowledge of USP 795 and FDA requirements of compounding and repackaging.
    • Operate the signature pad, cash register, payroll deduction, and credit card machines in order to promptly serve the customer.
    • Have a strong understanding of DAW codes, days' supply, origin codes, and ICD-10 codes.
    • Can properly re-bill previously checked out claims.
    • Identify opportunities through Outcomes.
    • Understand outpatient computer system functions including: refill too soon, IVR, on order, owe procedure, auto-refill function, prior authorization processing, and texting.
    • Contribute to audit prevention through training and prescription audits.
    • Familiar with the Vaccines for Children requirements and workflow.
    • Knowledge of how USP 800 affects nonsterile compounding and repackaging of hazardous medications.
  • Clinical Skills
    • Collaborate with patient, caregivers, and other healthcare professionals.
    • Keep up to date with current BLS modules.
    • Attend educational programs and read material relevant to pharmacy and healthcare to keep abreast of current information.
    • Maintain continuing pharmaceutical education requirements as mandated by the State Board of Pharmacy.
  • Leadership Skills
    • Oversee the duties of the technicians to ensure the team is working efficiently and effectively.
    • Work as a liaison between departments to communicate concerns, treatment concerns for patients, and issue resolution strategies.
    • Communicate appropriately and effectively to support the department mission and vision.
    • Create and maintain processes for efficient/effective team success.
    • Appropriately delegate work tasks to the pharmacy team to ensure a positive work environment.
    • Act as a role model for peers and team members by accepting responsibility for own behavior and performance.
    • Supportive of management and initiatives while providing counsel to peers having difficulty coping with change.
    • Possess a strong understanding of the core values and their importance to Children's strategy and be mindful to consider them when acting or making decisions, including displaying the appropriate behavior.
    • Collaborate with other healthcare professionals in a respectful manner to ultimately provide the best care for the patients and their families.
    • Perform other duties as assigned.
    Requirements
    • Bachelor's Degree in Pharmacy or PharmD Degree is required.
    • Rph Ohio - Registered pharmacist - Ohio State Board of Pharmacy is required.
