As a Physician Assistant, you will play a vital role in our mission to provide timely interventions, prevent avoidable emergency department visits, and alleviate social stressors that contribute to poor health.
Key Responsibilities:- Provide gap care visits in skilled facilities, acting as a primary care provider and treating patients in their homes.
- Makes facility rounds and performs patient exams, reviewing abnormal vitals and labs, and making recommendations to attending physicians.
- Reviews assigned new admissions and their orders, participating in readmission review meetings, and providing a summary of all PCMG acute discharges using the acute discharge worksheet.
- Functions independently to perform age-appropriate history and physicals for complex acute, critical, and chronically ill patients, developing and managing patient treatment plans.
- Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs, prescribes appropriate pharmacologic and non-pharmacologic treatment modalities, and implements interventions to support patients to regain or maintain physiologic stability.
- PA Degree from a College or University and two years related experience and/or training.
- Standing (1/3-2/3), walking (over 1/3-2/3), using hands to finger, handle or feel (over 2/3), reach with hands and arms (over 1/3), climb and balance (under 1/3), stoop, kneel (1/2-2/3), talk or hear (1/2-2/3), smell (under 1/3). Lifting up to 10 pounds (over 2/3), Up to 25 pounds (1/3-2/3); Up to 50 pounds (under 1/3)
