Job Summary:

Kaiser Permanente is seeking a skilled Clinical Social Worker to join our team. As a key member of our healthcare team, you will provide a wide range of clinical social work services to patients and their families, including support and consultation to care teams in managing patients. You will evaluate psychosocial assets and deficits to enable patients to maintain the greatest level of independence, coordinate with others to establish a plan of care, and provide short-term counseling, teaching, and brokering with other resources for caregiving needs.

Essential Responsibilities:
  • Provide appropriate social work interventions to all age patients and their families, including psychosocial assessment, brief individual or family counseling, advocacy, linkage, referral, group work, case coordination, and long-term care planning and placement.
  • Consult and coordinate with multidisciplinary teams of care professionals and primary care site medical/nursery staff, including family conferences, care conferences, team meetings, and other relevant activities.
  • Assess patient and family needs for care and communicate with other healthcare providers appropriately, collecting data relevant to visits and ongoing assessment throughout the duration of care.
  • Evaluate patients/family needs through timely assessment, communication, and observation, modifying the plan of care based on patient/care system response.
Basic Qualifications:
  • Minimum one (1) year of previous social work experience.
  • Masters degree in social work (MSW).
  • Licensed Marriage and Family Therapist (Washington) or Licensed Advanced Social Worker License (Washington) or Licensed Social Worker Associate-Independent Clinical (Washington) or Licensed Independent Clinical Social Worker (Washington) or Mental Health Counselor License (Washington).
Additional Requirements:
  • Excellent written and verbal communication skills.
  • Demonstrated success as part of a multi-disciplinary team and commitment to diversity.
Preferred Qualifications:
  • Two (2) years of post-masters experience.
  • Care coordination experience in managed care systems, in-/outpatient settings, skilled nursing, or long-term care facilities.
  • Knowledge of social case management, conflict resolution, and team building practices.
  • Familiarity with Microsoft Office and electronic charting.

Work Environment: This is a full-time position with a scheduled weekly hours of 40, working on-site at the Kaiser Permanente St. Joseph Medalia Building in Tacoma, Washington.

Pay Range: $59.9 per year, with the possibility of a higher salary based on location and experience.
