Job Summary

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
  • Develop and implement individualized plans of care, collaborating with the interdisciplinary team to prioritize patient and family needs
  • Provide end-of-life care, including comfort care and symptom management, according to established guidelines
  • Support and educate patients and families on disease processes, self-care techniques, and end-of-life care
  • Coordinate durable medical equipment and supplies as needed
  • Participate in quality assurance processes, internal education, and other activities promoting the organization's mission
Requirements
  • Graduation from an accredited school of nursing with a current license to practice as a registered professional nurse in Michigan
  • Minimum of one to two years of varied nursing experience, with hospice experience and CHPN certification preferred
  • Ability to use technology and communicate effectively with patients, families, and healthcare teams
  • Excellent interpersonal and organizational skills, with a commitment to confidentiality and positive relationships
