Job Summary

We are seeking a skilled Pediatric Private Duty LPN to provide high-quality nursing care to children in their homes. As a member of our team, you will work closely with patients, families, and healthcare professionals to deliver exceptional care and support.

Responsibilities
  • Provide direct nursing care to patients in their homes, following the plan of care and in coordination with the healthcare team.
  • Document patient visits thoroughly and accurately in the electronic medical record.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information and maintain a professional demeanor at all times.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
