About the Position

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments of patients in their homes, identifying potential health risks and developing personalized care plans to address them.

Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • Have a current BLS, ACLS, or CPR certification.
  • Have a minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant.
  • Have strong computer skills and familiarity with EMR software.
  • Possess excellent verbal and written communication skills with patients, clients, and colleagues.
  • Have a positive and outgoing personality and be a team player.
Travel Requirements

Have the ability to travel, a valid state driver's license, and be able to drive a car with proof of adequate automobile insurance coverage for the state of residence.

Travel required to meet patients where they live.
