About the Position

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time provider. In this role, you will conduct comprehensive health assessments, review medical histories, and provide health education to patients in their homes and other community environments.

Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and counseling to patients and their families
  • Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination
  • Work independently and as part of a team to achieve patient outcomes
Requirements
  • Masters degree in nursing or a related field
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
Travel Requirements

This position requires frequent travel to meet patients in their homes and other community environments. A valid state driver's license and proof of adequate automobile insurance coverage are required.
