Job Summary

The University of Kansas Health System is seeking a skilled Clinical Nurse Navigator to join our team. As a key member of our navigation team, you will play a critical role in promoting timely access to care and improving patient outcomes.

Key Responsibilities
  • Assess patient needs and develop personalized navigation plans to ensure timely access to care
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions between care settings
  • Provide education and support to patients and families to empower them to take an active role in their care
  • Develop and maintain relationships with referring physicians and other healthcare professionals to ensure effective communication and coordination of care
  • Participate in quality improvement initiatives to continuously improve patient outcomes and navigation processes
Requirements
  • Bachelor's degree in Nursing
  • 3+ years of nursing experience, with at least 1 year in case management, ambulatory care coordination, or a related field
  • Current RN licensure in the state of Kansas or multistate RN license
  • Basic Cardiac Life Support (BLS) certification
  • Specialized certification in the patient population being navigated (e.g. OCN)
