Key Responsibilities

  • Initiates, develops, implements, and revises the care plan in collaboration with physicians and other healthcare professionals involved in patient care.
  • Supervises care provided by home health aides and licensed practical nurses, offering guidance as needed and assigning tasks in accordance with state and federal regulations.
  • Delivers high-quality clinical services within the scope of practice and infection control standards, in alignment with the care plan and in coordination with the healthcare team.
  • Completes comprehensive assessments (OASIS) accurately and promptly, including medication reconciliation.
  • Conducts initial and comprehensive nursing evaluations, ensuring patients meet eligibility and medical necessity criteria as defined by payers, accurately determining the primary focus of care, developing the care plan in collaboration with the physician, and submitting precise documentation.
  • Communicates relevant information effectively and timely with agency staff regarding patient care issues, visit assignments, scheduled visits, and changes to the schedule, as well as coordinating care with other clinicians. Engages in timely communication with physicians, patients, and family members to ensure quality care and service excellence.

Qualifications

  • Current RN licensure in the state of Texas.
  • Current CPR certification.
  • Valid Driver's License, vehicle insurance, and access to a reliable vehicle.
