Key Responsibilities:

  • Perform advanced vocational nursing tasks to ensure the care, treatment, and overall welfare of individuals residing at the state-supported living centers, particularly those with complex medical, mental health, and/or behavioral health needs.
  • Conduct both routine and specialized nursing procedures to enhance the health of individuals within assigned areas and shifts.
  • Collect specimens for laboratory analysis and execute health screenings.
  • Provide guidance to direct support professionals and other staff on delivering care that meets the medical needs of individuals.
  • Carry out technical nursing procedures in compliance with facility and state regulations.
  • Stay informed and adhere to all agency, center, and unit policies and procedures.
  • Offer direction and assist with on-the-job training for new LVNs and direct care staff.
  • Support Registered Nurses (RNs) in implementing protocols and programs aimed at improving the health and safety of both staff and individuals.

This position is available for Morning, Evening, and Night shifts across various facilities.


Essential Job Functions:

  • Maintain regular attendance and may be required to work specific or rotating schedules, extended shifts, and/or overtime as per agency leave policies.
  • Administer medications and treatments via various methods, including oral, rectal, internal, or injection.
  • Assist other medical professionals, such as RNs, Physicians, and Advanced Practice Registered Nurses, in evaluating and caring for patients.
  • Conduct rounds and/or home visits as necessary and work in different locations as required.
  • Demonstrate proficiency in executing complex technical nursing tasks and procedures according to relevant regulations.
  • Train and instruct new LVN staff in nursing functions at the center, including familiarizing them with individual needs and preferences.
  • Perform intricate nursing procedures as mandated by facility and state regulations, including tracheostomy care, gastrostomy/jejunostomy care, wound care, and breathing treatments.
  • Monitor individuals' weight, diet, feeding, and vital signs, and assess health status post-treatment.
  • Document and report relevant treatments and observations in the individual's medical record accurately and promptly.
  • Facilitate interdisciplinary collaboration and communication among team members to ensure informed decision-making.

Knowledge, Skills, and Abilities:

  • Comprehensive understanding of nursing care standards, procedures, and techniques.
  • Familiarity with emergency protocols and equipment, including CPR, and the ability to assess and care for individuals in emergencies.
  • Proficient reasoning skills to identify problems and develop appropriate solutions.
  • Competence in operating computers and relevant software.
  • Ability to provide care to a diverse range of individuals with complex medical, mental health, and/or behavioral health issues.
  • Capability to maintain accurate records and utilize electronic health record systems post-training.
  • Effective communication skills to instruct and interact with individuals with intellectual/developmental disabilities and their families.
  • Current CPR certification is required, along with the physical ability to perform CPR safely.
