Job Overview:
Mental Health Partners is seeking an experienced Mental Health Case Manager Professional to join our Assertive Community Treatment (ACT) team. The ideal candidate will have a strong background in mental health, excellent communication and interpersonal skills, and the ability to work effectively in a team environment.

Responsibilities:
The Mental Health Case Manager Professional will be responsible for:
  • Providing assessment, planning, linkage, coaching, monitoring, supportive counseling, and advocacy services to clients with severe mental illness
  • Collaborating with the ACT team to develop and implement individualized treatment plans
  • Linking clients with community resources and services
  • Monitoring client progress and adjusting treatment plans as needed

Requirements:
To be successful in this role, you will need:
  • Bachelor's degree in Psychology, Social Work, Counseling, or related field
  • 2+ years of clinical experience, preferably in mental health
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Familiarity with community resources and services
