Overview

Novant Health is seeking a skilled and compassionate Registered Nurse to join our Home Health team at New Hanover Regional Medical Center in Wilmington, NC. As a key member of our team, you will be responsible for coordinating home care services, establishing and maintaining patient plans of care, and ensuring the quality, customer service, and financial management of your assigned caseload.

Responsibilities
  • Conduct admissions, recerts, ROC, and discharges for assigned caseload
  • Perform routine nursing visits as needed
  • Assess patient needs and coordinate care with patients, caregivers, physicians, and other disciplines
  • Complete all home health documentation
Qualifications
  • Graduate of an accredited school of nursing required
  • Associate's or Bachelor's degree in nursing
  • Current valid North Carolina nursing license required
  • Basic Life Support (BLS) through American Red Cross or American Heart Association required
  • Driver's License Class C
  • Minimum 1 year of home health or related experience required
