Key Responsibilities:
- Oversee and manage clinical care for residents in skilled nursing facilities (SNFs).
- Conduct daily rounds and prescribe necessary medications and treatments.
- Deliver high-quality, patient-centered primary care services.
- Ensure timely documentation of all patient interactions.
- Be available for consultations with nursing staff via phone or secure messaging.
- Complete documentation using Theoria's proprietary electronic health record (EHR) system, ChartEasy.
- Familiarity with PointClickCare, MatrixCare, and American HealthTech is beneficial.
- Maintain punctuality for scheduled shifts and reliable transportation is required.

Shift Structure:
Rounding occurs from 7 AM to 1 PM local time, during which providers must be accessible to nursing staff and address any urgent issues.
Shifts are scheduled Monday through Friday, with flexibility available.
Documentation can be completed off-site, although this is an in-person role.
