Responsibilities:
* Provide basic nursing services to patients in their homes
* Conduct home visits to assess patient needs and develop care plans
* Administer medications and treatments as prescribed by physicians
* Monitor patient progress and report any changes or concerns to healthcare teams
Requirements:
* Current Massachusetts LPN license
* 1-2 years of experience as an LPN
* 1-2 years of recent Medicare certified home care experience or related health field
* Knowledge of Medicare Conditions of Participation
* Understanding of OASIS documentation
* Valid drivers license and reliable transportation
Benefits:
