About the Role

As a Home Health Registered Nurse with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes. Our team is dedicated to providing comprehensive care that addresses the physical, emotional, and social needs of our patients.

Key Responsibilities
  • Provide skilled nursing visits to home health patients, including admission, case management, and follow-up care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Confer with physicians to develop and revise care plans based on ongoing assessments and patient progress.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Monitor patient progress, report to physicians and Clinical Managers, and revise care plans as needed.
  • Supervise and educate home health aides and personal care workers to ensure high-quality care.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in home health or a related field.
  • Strong med-surg, ICU, ER, and acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
