Job Title: Registered Nurse - Home Health

BAYADA Home Health Care is seeking a skilled and compassionate Registered Nurse (RN) to join our team in providing high-quality home health care services to our clients in Plymouth, New Hampshire.

About the Role:
  • As a Registered Nurse - Home Health, you will be responsible for delivering patient-centered care to clients in their homes, focusing on their unique needs and goals.
  • You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring seamless coordination of care.
  • With a strong emphasis on patient safety and quality of care, you will monitor and report any changes in client condition, collaborating with healthcare professionals as needed.
  • Our RNs are empowered to make decisions and take ownership of their practice, ensuring the best possible outcomes for our clients.
Responsibilities:
  • Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Develop and implement individualized care plans, incorporating the client's goals, preferences, and values.
  • Provide skilled nursing care, including wound care, medication management, and education on self-care techniques.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document client progress, including assessments, interventions, and outcomes, in a timely and accurate manner.
Requirements:
  • A current license as a Registered Nurse in New Hampshire.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills, with the ability to communicate effectively with clients, families, and healthcare professionals.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
