Physiatrist Position

We are seeking a skilled Physical Therapist to join our team at HCA Virginia Healthcare at Home. As a PRN Physical Therapist Home Health, you will play a vital role in providing high-quality patient care and support throughout the continuum of care.

The Physical Therapist will be responsible for managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team. This includes providing or directing a majority of the patient's physical therapy to support patients throughout their recovery at home.

• Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• Develop and implement individualized plans of care for patients under the supervision of a physician.
• Collaborate with the patient care team to ensure seamless care coordination and communication.
• Provide education and support to patients and their families to promote optimal health outcomes.
