The Care Coordination Team Leader plays a crucial role in upholding the high-fidelity wraparound model of care coordination, ensuring quality support for children and families from diverse backgrounds. This role requires a comprehensive understanding of local resources and the ability to cultivate strong relationships with staff, clients, and community stakeholders. The position oversees services in multiple counties.

Key Responsibilities:

  • Supervise and guide the care coordination team through various methods, including team meetings and regular communication.
  • Provide training and support for direct care coordinators in both home and community settings.
  • Facilitate ongoing training and assistance to staff through individual and group discussions, aiding in case analysis and skill enhancement.
  • Foster a culture of recognition and teamwork among team members.
  • Maintain high fidelity wraparound training credentials.
  • Ensure the completion of necessary training for supervising ICC and MCC delivery, including reviewing and approving documentation such as care plans and assessments.
  • Oversee the coordination of on-call rotations.
  • Engage in continuous fidelity reviews and monitoring to ensure adherence to system of care principles.
  • Collaborate with relevant systems and key participants to guarantee high-quality services, emphasizing family involvement.
  • Assess needs during crisis situations and inform appropriate responses.
  • Maintain confidentiality and ensure compliance with quality standards.
  • Evaluate staff performance and recommend actions to management.
  • Participate in the recruitment process for new staff, including reviewing applications and conducting interviews.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's or master's degree in social work, education, counseling, psychology, or a related field is preferred.
  • Licensure as an LSW or LPC is preferred.
  • A minimum of three years of experience in providing coordinated supportive services.
  • Previous supervisory experience is preferred.
  • Experience in the behavioral health field is required, particularly with multi-system youth or individuals with developmental disabilities.
  • Familiarity with local, state, and federal regulations regarding behavioral health services is essential.
  • Knowledge of high-fidelity wraparound services is preferred (training will be provided post-hire).
  • Strong communication skills, both verbal and written, are necessary for effective interaction with clients and colleagues.
  • Excellent organizational skills and the ability to manage multiple tasks are required.
  • Demonstrated clinical knowledge of children with complex behavioral health issues is essential.
  • Experience with Electronic Health Record systems is beneficial.
