Healogics is seeking a skilled RN Case Manager PT to join our team. As a key member of our Wound Care Center, you will provide exceptional patient care and handle intake and discharge processes with precision and compassion.
Key Responsibilities:- Provide patient care to Wound Care Center patients and handle patient intake and discharge processes as assigned and according to hospital and nursing standards.
- Assist in vein clinic as assigned after Healogics Vein Clinic training completed.
- May also serve as Case Manager to a group of patients as assigned, performing initial nursing assessments, communicating findings with assigned physicians, and documenting care plans.
- Participate in quality improvement initiatives, provide education to patients or caregivers, and serve as a patient advocate in the delivery and coordination of patient care.
- Perform hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of Healogics hyperbaric training.
- Degree in Nursing (ADN)
- Current RN license in the state of practice - to be maintained throughout the duration of employment in the position
- Bachelor of Science in Nursing (BSN) preferred
- Medical Surgical, Home Health or Wound Care Certification preferred
- Wound care experience preferred
The hourly rate for this position generally ranges between $40.72-$52.36 Per Hour. This range is an estimate, based on potential employee qualifications: education, experience, geography as well as operational needs and other considerations permitted by law.
