About the Role

As a Physical Therapist at AdvantageCare Rehabilitation, you will play a vital role in delivering high-quality patient care and contributing to our mission of providing exceptional healthcare services.

Key Responsibilities
  • Conduct Comprehensive Evaluations: Assess patients' physical conditions, functional limitations, and mobility to develop effective treatment plans.
  • Develop Individualized Treatment Plans: Create personalized plans based on clinical assessment and patient goals to ensure optimal outcomes.
  • Utilize Therapeutic Techniques: Employ various techniques, including exercises, manual therapy, and modalities, to promote patient recovery and well-being.
  • Maintain Accurate Documentation: Record patient evaluations, treatment plans, progress notes, and discharge summaries in a timely and accurate manner.
  • Communicate Effectively: Collaborate with healthcare professionals, patients, and families to ensure seamless communication and coordination of care.
  • Educate Patients and Families: Provide education on the therapeutic process, home exercise programs, and injury prevention to empower patients and their loved ones.
  • Work Collaboratively: Partner with interdisciplinary teams to ensure comprehensive patient care and achieve optimal outcomes.
  • Participate in Quality Improvement: Contribute to maintaining high standards of patient care and participate in quality improvement initiatives.
Qualifications
  • Graduate of a Physical Therapy Curriculum: Hold a degree from a physical therapy program accredited by the American Physical Therapy Association (APTA) or Commission on Accreditation in Physical Therapy (CAPT).
  • Valid State Physical Therapy License: Possess a current and valid state physical therapy license.
