Delaware RN Case Manager

We are seeking a skilled Registered Nurse Case Manager to join our team in Delaware. This full-time, 3-month contract-to-hire position offers a unique opportunity to make a meaningful impact in the lives of our clients.
Job Summary
As a Registered Nurse Case Manager, you will provide community-based care coordination and management for our clients in Delaware. Your primary responsibility will be to assess, plan, coordinate, implement, and evaluate care for eligible members with complex health needs. You will work closely with healthcare providers, social services, and other stakeholders to ensure that our clients receive the highest quality care possible.
Your Key Responsibilities
• Conduct face-to-face needs assessments with clients and their caregivers
• Develop and implement individualized care plans that meet the unique needs of each client
• Coordinate care across multiple settings, including hospitals, nursing facilities, and home-based care
• Facilitate transitions between care settings and ensure seamless continuity of care
• Educate clients and their caregivers about available resources and services
• Collaborate with healthcare providers and other stakeholders to ensure that clients receive comprehensive care
• Document all case management activities and interventions in our electronic health record system
Requirements
To be successful in this role, you will need:
• A current registered nurse license in the state of Delaware
• At least 5 years of experience in case management and discharge planning, with a focus on geriatric populations
• Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
• Proficiency in electronic documentation systems and Microsoft Office software
• Reliable transportation and the ability to travel within assigned territories
• Ability to work flexible hours, including evenings and weekends as needed
Preferred Qualifications
• Certified Case Manager (CCM) certification
• Experience working with HIV/AIDS, behavioral health, or developmental disabilities populations
• Familiarity with Medicare and Medicaid regulations
• Bachelor's degree in nursing or related field
