About the Role

We are seeking a highly skilled Nurse Coordinator to join our team in Delaware. In this role, you will provide community-based care coordination and management services to members, ensuring they receive the best possible healthcare outcomes.

Key Responsibilities
  • Conduct face-to-face needs assessments with members in their homes, nursing facilities, or other community-based settings.
  • Develop and implement personalized care plans to address members' chronic and complex healthcare needs.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal health outcomes.
  • Educate members and caregivers on healthcare options, resources, and benefits to empower informed decision-making.
Requirements
  • Current Registered Nurse licensure in the state of Delaware.
  • At least 5 years of experience in case management and discharge planning, including facility discharges.
  • Strong assessment and care planning skills, with experience working with geriatric special needs, behavioral health, and home health populations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
What We Offer

This is a full-time, 3-month contract-to-hire role with hybrid work arrangements. The successful candidate will enjoy a competitive salary range of $60,000 - $80,000 per year, depending on experience.
