We are looking for a dynamic and mission-driven nurse leader who is passionate about improving the quality of life for the individuals we serve.
Key Responsibilities- Monitor physician and ancillary visits and orders
- Address resident observations and concerns with the appropriate coordinators
- Provide guidance and support to nursing staff as necessary
- Oversee discharge planning for residents, including arranging home services
- Manage certifications and recertifications for residents in skilled nursing
- Facilitate weekly meetings related to Medicare, therapy, and resident care
- Act as a liaison for therapy services
- Conduct reassessments for residents and manage hospital visits when needed
- Support the restorative care program
- Participate in daily stand-up meetings
- Assist in managing the survey process
- Support physician rounds
- Serve as the on-call nurse leader on a rotating schedule
- Perform additional duties as assigned.
- Education: Graduate of an accredited nursing program
- Licensure/Certification: Current Ohio Registered Nurse (RN) license
- Experience: Minimum of three years of nursing experience in long-term care; experience in restorative nursing or as a nursing supervisor is preferred.
- Computer Skills: Proficient in electronic medical record systems and documentation.
- Detail-oriented with a strong ability to lead and deliver high-quality service in a resident-focused environment.
- Knowledgeable about clinical standards, regulations, and reimbursement policies relevant to long-term care and Medicare/Medicaid.
- Ability to recognize and uphold the autonomy and dignity of all residents.
- Effective communication skills that convey care, concern, and respect for residents.
- Skilled in empowering residents to make decisions regarding their care.
- Capable of making timely and accurate judgments in resident care and emergency situations.
