Key Responsibilities:
The Hospice Aide serves as a vital member of the home care team, operating under the guidance of a registered nurse or therapist. This role involves providing essential personal care services tailored to meet the individual needs of patients. The Hospice Aide is tasked with observing patients, documenting care provided, and reporting any significant changes in their condition.

Essential Duties Include:
1. Delivering personal care services such as:
A. Bathing
B. Back massages
C. Oral hygiene assistance
D. Hair washing
E. Changing bed linens
F. Aiding patients with dressing and undressing
G. Skin care to prevent breakdown
H. Supporting toileting needs
I. Maintaining a clean and orderly living environment for patients
2. Planning and preparing nutritious meals.
3. Assisting with feeding when necessary.
4. Monitoring and recording vital signs as directed (with appropriate training and competency).
5. Aiding in patient mobility and exercise as per care plans.
6. Performing range of motion exercises and other basic procedures as directed (with appropriate training and competency).
7. Supporting patients in self-administering medications.
8. Managing laundry tasks for patients as needed.
9. Ensuring patient safety and proper use of equipment (e.g., foot stools, side rails).
10. Reporting on patient conditions and any notable changes to the assigned nurse.
11. Complying with the organization's documentation and care standards, along with maintaining professional conduct.

Qualifications:

  • 1. Completion of training requirements as per State and Federal regulations.
    2. Preferred registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau.
    3. Minimum age of 18 years.
    4. Ability to read and follow written instructions and document care provided.
    5. Self-motivated with the capability to work independently.
    6. Empathy towards the needs of the ill, injured, and frail.
    7. Current CPR certification is required.
    8. Demonstrated patience, tact, and good personal hygiene.
    9. Valid driver's license with an insured vehicle in good working condition.
  • Experience in Home Health or Hospice is advantageous.
