Job Title: LPN Direct Service Provider

We are seeking a skilled LPN to join our team as a Direct Service Provider. In this role, you will provide high-quality healthcare services to individuals with developmental disabilities living in community-based homes.

Key Responsibilities:
  • Monitor and maintain the delivery of preventive and routine health care services for individuals with developmental disabilities.
  • Provide preventative, routine, and emergency healthcare services as needed.
  • Ensure compliance with all applicable state and federal regulations, as well as agency policies and procedures.
  • Communicate effectively with site RNs, LPN Senior Residential Supervisors, and other staff members as necessary.
  • Assist individuals with daily living activities and provide support as needed.
  • Complete all necessary documentation and reports in a timely manner.
Requirements:
  • Current NYS license to practice as a Licensed Practical Nurse.
  • Valid Driver's License that meets agency policy.
  • CPR certification.
  • Ability to meet physical requirements of the position, including standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
