About the Role

We are seeking a skilled MDS Coordinator to join our team at Diversicare Healthcare Services & Diversicare Ther. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality care for our patients and residents.

Responsibilities

  1. Facilitate the RAI Process under the direction of an R.N. or DNS.
  2. Work in collaboration with the interdisciplinary team to identify the needs of the patient/resident.
  3. Coordinate and conduct the daily Case Management meeting.
  4. Ensure timely MDS assessments according to state and federal regulations.
  5. Ensure interdisciplinary team completes designated sections of the MDS assessment.
  6. Ensure accurate coding of the MDS assessment with information obtained via medical record review, observation, and interview with center staff, patients, residents, and family members.
  7. Ensure documentation is available to support accurate coding of the MDS assessment.
  8. Maintain the tracking system of MDS assessment schedules (timeframes and due dates).
  9. Facilitate Care Plan conferences with the interdisciplinary team, patient, residents, and families.
  10. Obtain, review, and maintain all State and Federal reports, making appropriate corrections timely.

Requirements

  1. Must hold current nursing license in the state of employment.
  2. Experience in Long Term Care.
  3. Computer skills.
  4. Knowledge of Medicare, Managed Care, and RAI Process Required.
  5. Participating in the LPN to RN Back to School transition program.
