Position Overview

ROLE SUMMARY: The Triage Practice Nurse is responsible for delivering nursing care to patients within a healthcare setting. This role encompasses providing care to individuals across various age groups: pediatric (infants to 12 years), adolescent (13 to 17 years), adult (18 to 64 years), and geriatric (65 years and older).

KEY RESPONSIBILITIES: Candidates must possess the capability (with or without reasonable accommodation) to execute the following tasks:
  • Deliver clinical guidance via phone or electronic communication, adhering to clinical protocols and regulatory standards. Collaborate with healthcare providers as needed.
  • Offer tailored patient education on topics such as preventive care, chronic disease management, acute conditions, medications, and other relevant areas impacting patient outcomes.
  • Prepare patients for consultations by utilizing established rooming standards.
  • Administer prescribed injections and medications, following departmental protocols and regulatory guidelines.
  • Assist healthcare providers with various procedures, minor surgeries, and examinations as directed.
  • Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
  • Collect blood and other laboratory specimens, ensuring proper preparation for further analysis.
  • Authorize prescription refills as per provider instructions.
  • Support test tracking and monitor overdue results.
  • Perform suture removal and dressing changes as required.
  • Ensure examination rooms are well-stocked and sanitized after each patient visit. Maintain and clean medical instruments and equipment post-procedure.
  • Assist providers and patients in reviewing and updating care plans, evaluating patient progress, and following up as necessary.
  • Engage in proactive outreach for patients needing preventive and chronic disease services.
  • Facilitate care coordination among specialists and manage transitions of care to gather all essential patient information.
  • Support the coordination of referrals, education, and community resources.
  • Actively participate in initiatives aimed at enhancing patient experience and quality of care.
  • Engage in daily team meetings with healthcare providers.
  • Serve as an advocate for patient experience, maintaining a positive demeanor in all interactions with patients, visitors, and colleagues. Communicate professionally while prioritizing customer service.
  • Handle all patient information confidentially, in accordance with privacy policies and procedures.

ADDITIONAL RESPONSIBILITIES: The following tasks are considered secondary to the primary responsibilities listed above:
  • Perform various administrative duties as needed.
  • May require occasional travel; a valid driver's license is necessary for travel.
  • Participate in on-call responsibilities as required.
  • Other duties as assigned.

QUALIFICATIONS:

MINIMUM REQUIRED:
  • Completion of a Licensed Practical Nurse (LPN) or Registered Nurse (RN) educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the appropriate state board.
  • AHA CPR/BLS certification required within 30 days of employment.
  • This position may require occasional travel to multiple sites.

PREFERRED QUALIFICATIONS:

  • One (1) to Two (2) years of nursing experience in a primary or specialty care setting.
  • Previous experience in triage, emergency care, cardiac care, intensive care, or related nursing specialties.
