Job Summary

The Rosemont Health & Rehab Center is seeking a skilled Licensed Social Services Worker to join our team. As a key member of our Social Services Department, you will play a vital role in ensuring the emotional and social needs of our residents are met.

Key Responsibilities
  • Assist the Director in planning, organizing, and implementing the Social Services Department's operations in accordance with federal, state, and local standards.
  • Develop and maintain relationships with residents, families, and healthcare professionals to ensure comprehensive care.
  • Conduct assessments, develop care plans, and provide counseling services to residents as needed.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
  • Maintain accurate and detailed records of resident interactions, progress, and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • High school diploma or equivalent required; degree in social work or related field preferred.
  • Current, unencumbered license to practice as a Social Worker in the state where you will work.
  • Excellent communication and interpersonal skills; ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility to work varying shifts, including evenings and weekends, as needed.
