Job Summary

We are seeking a highly skilled and experienced Nurse Supervisor/LPN to join our team at StoryPoint of Schererville. As a key member of our healthcare team, you will be responsible for leading, managing, and training a team of caregivers to provide exceptional care to our residents.

Key Responsibilities:

  • Lead and manage a team of caregivers to ensure high-quality care and services are provided to residents.
  • Evaluate residents and report any changes in condition to physicians in a timely and accurate manner.
  • Utilize Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR) systems to document resident care and medication administration.
  • Communicate effectively with team members, residents, and families to ensure seamless care coordination.
  • Build strong relationships with residents, families, and team members to promote a positive and supportive work environment.

Requirements:

  • Current licensure as a Licensed Practical Nurse (LPN) in the state.
  • Assisted Living/Memory Care experience preferred.
  • Prior supervisory experience required.
  • Punctuality and professionalism are essential.
  • Passion for working with the elderly and promoting their well-being.
