Job Title: Triage Registered Nurse

Job Summary:

The Triage Registered Nurse is a critical member of our team at Community Healthcore, responsible for providing high-quality nursing care to individuals with mental illness and/or co-occurring substance use/abuse diagnoses. This role requires a strong understanding of nursing standards and the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Utilize the nursing process to plan, implement, and document nursing care provided to clients under the requirements of the Texas Nurse Practice Act and in accordance with nursing licensure standards and established policies and procedures of Community Healthcore.
  • Conduct off-site and on-site triage, crisis screening, crisis intervention, service coordination, ANSA/CANS, and other ancillary services as indicated for clients presenting with a mental health and/or a co-occurring substance use/abuse diagnosis.
  • Work to ensure the safety of individuals, staff, and visitors.
  • Collaborate with the Director of Nursing (DON) and the Regional Crisis Response Center (RCSU) RN Nurse Manager to meet the needs of individuals in the RCRC.

Requirements:

  • Graduated from an accredited school of nursing.
  • Licensed to practice as a Registered Nurse in the State of Texas.
  • Current CPR certification.
  • Current valid driver's license in state of residence, proof of automobile liability insurance, and a driving record insurable by the center's insurance carrier.
  • Demonstrates knowledge of standards of nursing practice for Registered Nurses.
  • Associate Degree in Nursing (ADN).
  • Working knowledge of licensing standards for Crisis Stabilization Units.
  • 1 year experience preferred in Nursing/Mental Health.

Physical and Mental Requirements:

  • Ability to exercise self-control in potentially volatile situations.
  • Must respond quickly and effectively.
  • Must be able to work and concentrate amidst distractions such as noise, conversations, and foot traffic.
  • Must be flexible and able to complete tasks assigned in a timely manner.
  • Must be able to stoop, kneel, crouch, reach, stand, walk, push, pull, lift, grasp, and display accurate perception of objects (size, shape, temperature, texture).
  • Must be able to perceive detailed information through written and oral communication.
  • Must be able to carry charts and assist with patients (up to 50 lbs.).
  • Must be able to verbalize information and respond to complaints.
  • Must have minimum standard of visual acuity with or without correction that enables completion of tasks/duties and visual observation of patients.
  • Must be able to attend meetings.
  • Must have endurance to work an average 8-hour day - 12 hours may be required on isolated occasions.
