About the Role

We are seeking a highly skilled Licensed Vocational Nurse III to join our Hospice Care team at Kaiser Permanente. As a key member of our team, you will provide high-quality patient care and support to our patients and their families.

Key Responsibilities
  • Provide Direct Patient Care: Under the supervision of a Registered Nurse or physician, you will provide nursing care to patients in accordance with established standards of care, policies, and procedures.
  • Assist with Medical Procedures: You will assist medical personnel with special procedures, minor surgical procedures, and diagnostic exams.
  • Monitor Patient Condition: You will assess and monitor patients' condition and notify the physician or healthcare professional for disposition.
  • Administer Medications: You will prepare and administer medications as directed by physician order following established protocols.
  • Maintain Equipment and Supplies: You will maintain equipment and supplies, identify and arrange for equipment needing repair, and apply principles of aseptic technique and infection control.
  • Provide Medical Information and Education: You will provide medical information and education to patients, following established protocols and guidelines.
  • Document Patient Information: You will document pertinent patient information, assessment, and nursing procedures following established protocols.
  • Perform Other Duties: You will perform other duties as directed by your supervisor.
Requirements
  • Experience: Minimum one (1) year of recent (within the last three (3) years) experience as a LVN in a home health/hospice or acute care setting.
  • License, Certification, Registration: Vocational Nurse License (California), I.V. Certification, and Basic Life Support.
