Key Responsibilities: In this position, you will deliver direct nursing care to patients in their residences or care facilities. Your responsibilities include:
- Actively listening to patients' needs and respecting their preferences.
- Building and maintaining trusting relationships with patients and their families.
- Serving as the primary communication link within our interdisciplinary team of holistic care providers.
Qualifications:
- A minimum of 6 months of prior skilled nursing experience.
- An active, unencumbered state Registered Nurse license.
- A valid driver's license and auto liability coverage.
- A genuine interest and capability in providing patient care in home and facility settings.
