RN Care Manager Ambulatory Specialist

We are seeking a skilled RN Care Manager to join our team at SCL Health. As a RN Care Manager, you will play a critical role in improving the health of patients with chronic conditions or complex needs.
Key Responsibilities- Coordinate care and services for patients identified as needing assistance or meeting Care Management criteria.
- Collect in-depth information about a patient's medical, functional, and social condition to identify individual needs and develop a plan to meet those needs.
- Work with patients and families/caregivers to determine specific goals and actions based on assessment, coordinate discharge planning, and execute specific interventions to meet established goals.
- Assess the patient's prior level of functioning, access to and/or use of community resources, and available support systems.
- Assist the care team in developing a plan of care that includes ensuring appropriateness of services and care setting, individualized support and education, determining the need for continued services, planning for discharge, and identifying and connecting patients/families with available community resources if needed.
- Collaborate with physicians and other members of the health care team on the patient's behalf.
- Identify appropriate admission and continued stay issues, enhance the quality of patient care through effective and efficient use of resources, and help identify strategies for reducing length of stay and appropriate utilization of services.
- Attend, facilitate, and participate in rounds and case conferences, advocate for patient rights, monitor efficiency and availability of services, and evaluate outcomes through variance tracking and data collection.
- Identify needs, facilitate, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles, and participate in multidisciplinary groups and development of guidelines.
- Promote mission, vision, and values of SCL Health, and abide by service behavior standards.
- Graduate of an accredited school of nursing is required.
- Current valid State RN License is required.
- Three (3) years of experience as a Registered Nurse is required, acute care setting required.
- BSN preferred.
- Five (5) years nursing experience preferred.
- Ongoing need for employees to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- May be expected to stand in a stationary position for an extended period.
