Chronic Disease Prevention Program Manager

- Maintain accurate electronic records of referred individuals and their health information
- Conduct motivational interviewing sessions to promote healthy behaviors and disease management
- Assist with program marketing and retention efforts to increase enrollment
- Ensure timely completion of intake forms, data collection forms, and program documentation
- Bachelor's degree in health promotion, public health, or a related field
- At least one year of experience in health education or a related field
- Excellent communication and interpersonal skills
- Intermediate to advanced computer skills and experience with Microsoft Office
- A competitive salary range of $22-$24.20/hour
