Job Title: Licensed Practical Nurse - Private Duty Nursing

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelliChoice Home Care in Greensboro, NC. As a Private Duty LPN, you will provide one-on-one nursing care to critically ill patients in their homes, focusing on ventilator and tracheostomy care.

Responsibilities:
  • Provide high-quality, patient-centered care in the home setting
  • Assess and manage patients with complex medical conditions, including ventilator and tracheostomy care
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans
  • Administer medications, perform treatments, and provide wound care as needed
  • Document patient progress and communicate effectively with healthcare teams
Requirements:
  • Current LPN license in the state of North Carolina
  • CPR certification (in-person training only)
  • TB skin test (current within the last 12 months)
