LVN Wellness Case Manager

InGenesis is seeking a skilled LVN Wellness Case Manager to join our team. As a key member of our healthcare staff, you will play a vital role in providing exceptional patient care and support.
Job Summary:The LVN Wellness Case Manager will be responsible for evaluating, planning, and providing case management services, health education, and counseling services for patients with complex chronic conditions. This role requires strong analytical and communication skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Evaluate patient needs and develop individualized care plans to address complex chronic conditions.
- Provide health education and counseling services to patients and their families.
- Maintain accurate and up-to-date patient records, including chronic disease registries and lab results.
- Coordinate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Develop and implement strategies to improve patient engagement and adherence to treatment plans.
- Collaborate with multidisciplinary teams to address patient needs and improve overall care quality.
- High School Diploma or equivalent.
- Completion of LVN program.
- Current license to practice as an LVN in California.
- 1-2 years of work-related experience in a medical environment.
- Active phlebotomy certificate/blood draw certification.
- Access to reliable transportation.
- Proficiency in Microsoft Office and the internet.
- Associate or bachelor's degree in a related field.
- Experience working in a community health center, long-term care facility, or public health setting.
- Experience working with patients with chronic diseases, such as diabetes and hypertension.
- Outreach and presentation experience.
