We are seeking an exceptional Home Health Physical Therapy Assistant to join our team in Danville, VA.

About the Role:

This position offers a unique blend of clinical expertise and patient-centered care in a dynamic home health environment.

Main Responsibilities:

  • Deliver high-quality physical therapy services to patients in their homes, promoting independence and well-being.
  • Collaborate with healthcare teams to develop and implement comprehensive care plans tailored to individual patient needs.
  • Provide exercise instruction, patient education, and guidance on safe mobility and daily activities.
  • Record and report patient progress, identifying areas for improvement and making recommendations for treatment adjustments.
  • Work closely with supervising Physical Therapists to ensure seamless care coordination and adherence to established plans.

Requirements:

  • Current licensure as a Physical Therapy Assistant (PTA) in the state of practice.
  • CPR certification is required.
  • Valid driver's license, vehicle insurance, and reliable transportation.
