Quality Improvement Nurse

The Quality Improvement Nurse is a key member of the management team at WMCHealth, responsible for overseeing the agency's QAPI Plan, Risk Management, and Infection Control programs. This role ensures compliance with CHAP, Federal, and New York State regulatory standards and requirements.
As an educator, the Quality Improvement Nurse evaluates staff educational needs and develops and provides educational programs based on clinical needs and interests. This includes integrating current home care regulatory and evidence-based best practice knowledge when orienting staff or providing direct care.
The Quality Improvement Nurse is responsible for analyzing data for trending and process improvement and utilization review. This role also meets all of the Principle Duties and Responsibilities of the Registered Nurse and has a visit component expectation.
Responsibilities- Oversee the quality assurance performance improvement program for home care, including quality management of OASIS data and reporting, process and outcome reports, occurrence reports, home health compare and patient satisfaction data, program quality monitoring data, utilization data, payer inquiry requests, denial activity, fall data, and infection control practices.
- Use data trending to initiate and sustain performance improvement activities.
- Coordinate findings with clinical and finance leadership.
- Serve as a regulatory resource for Federal, State, and CHAP regulations and standards.
- Develop, monitor, and evaluate corrective action plans for deficiencies identified through the quality review process and survey process.
- Coordinate findings with billing and finance as appropriate.
- Responsible for chart workflow processing and review of documentation for accurate submission of OASIS data to CMS for Start of Care admissions, Resumption of Care, and Recertification information aligned with condition of Participation and Conditions for billing.
- Responsible for compiling data reports and data collection for monthly audits to ensure compliance with Agency policies and procedures, CHAP Accreditation, NYS state, and federal regulations.
- Oversee the Fall and Infection Control programs to identify and data trend with performance improvement recommendations to reduce adverse event and rehospitalization data reports.
- Evaluate the need for and plan and coordinate special classes, conferences, and meetings to improve the level of staff performance and quality of care.
- Participate in the weekend and holiday clinical supervisor schedule.
- Participate in WMCHealth MidHudson Regional hospital committees.
- Fulfill all compliance responsibilities related to the position.
- Develop, conduct, and evaluate programs that assist in meeting the educational needs of home care clinical staff competency.
- Assist in the development, implementation, interpretation, and evaluation of educational training program standards, policies, procedures, and best practices appropriate to learner needs, desired outcomes, content, and context.
- Utilize learning technologies to develop, launch, and monitor instructor-led, in-service, and online learning as assigned.
- Collaborate with nursing leadership to provide input into the evaluation of staff performance and readiness to assume staff responsibilities post-educational initiatives.
- Conduct field supervisory visits and home visits with clinical staff to ensure a competent workforce.
- Perform other duties as required.
A minimum of two years of home care practice setting required.
Graduate of an accredited school of nursing required.
Current NYS RN license required.
Valid NY State Driver's License.
Understands Home Care regulations Conditions of Participation.
