The Senior Clinical Pharmacist is tasked with delivering comprehensive pharmaceutical care to patients within the Atrium Health system, acting as the primary representative for pharmacy services. Key responsibilities encompass overseeing the delivery of pharmaceutical care, ensuring that pharmacy systems align with patient needs, and managing both quality and financial outcomes.

Core Responsibilities:

1. Deliver pharmaceutical care to patients in designated areas.

2. Formulate pharmaceutical care strategies, protocols, and delivery methods that align with therapeutic objectives and enhance patient quality of life.

3. Conduct consultations regarding pharmacokinetic dosing, pharmacotherapy, parenteral nutrition, and other collaborative drug therapy practices as per departmental guidelines.

4. Maintain accurate documentation of consultations and follow-ups in the medical records in accordance with professional standards.

5. Monitor patient progress relative to the pharmaceutical care strategy.

6. Intervene as necessary to guarantee safe and effective medication therapy outcomes.

7. Collaborate with prescribers and other healthcare professionals to address specific patient needs related to medication management.

8. Document outcomes of pharmacist interventions.

9. Supervise Clinical Pharmacist I in monitoring patient activities.

10. Engage in medical care rounds, transitional care planning, multidisciplinary rounds, and critical path development.

11. Serve as the primary liaison for patients, families, and other healthcare professionals regarding pharmaceutical care.

12. Oversee pharmacy systems and financial performance for assigned areas.

13. Act as the pharmacy service representative to medical, nursing, and other disciplines concerning pharmacy services.

Qualifications:

  • Pharm.D., M.S., or B.S. Degree with a minimum of 2 years of relevant clinical practice experience.
  • At least 1 year of experience in a health system with a focus on hospital practice.
  • Possession of a valid Georgia Pharmacist License.
