We are seeking a highly skilled Clinical Pharmacist to join our team at Atrium Health. As a key member of our pharmacy team, you will be responsible for providing pharmaceutical care to patients and serving as the service representative for Pharmacy.
Key Responsibilities- Develop and implement pharmaceutical care plans, protocols, and care delivery to meet therapeutic goals and improve quality of life for patients.
- Perform consultations in pharmacokinetic dosing, pharmacotherapy, and parenteral nutrition, and other collaborative drug therapy practice as defined by department/organization policy.
- Document consults and follow-ups in medical records according to department/professional standards.
- Participate in medical care rounds, transitional care rounds, multidisciplinary rounds, and critical path development.
- Manage pharmacy systems and financial performance for assigned areas and serve as the pharmacy service representative to medical, nursing, and other disciplines regarding pharmacy services.
- Pharm.D, M.S., or B.S. Degree
- One year previous health system experience with emphasis on hospital practice preferred
- Valid Georgia Pharmacist License
