About the Role

We are seeking a compassionate and highly skilled Licensed Practical Nurse to join our team in Yamhill County, Oregon. As a member of our interdisciplinary team, you will play a critical role in delivering high-quality healthcare services to individuals with serious mental illness.

Job Responsibilities

  • Conduct thorough assessments and develop comprehensive treatment plans
  • Provide general and specialized nursing services, including medication administration and monitoring
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
  • Develop and implement individualized treatment plans to promote recovery and wellness

Requirements

  • Currenly licensed to practice as a Licensed Practical Nurse in the State of Oregon
  • Two years' experience as an LPN preferred
  • New graduates welcome to apply
  • A National Provider Number (NPI) or ability to obtain one upon employment
  • A valid license to drive in Oregon and a good driving record
